What to Bring to Your Appointment
If you are a new patient, please plan on arriving at least 15 minutes early. Please bring the following items with you:
- Insurance card(s)
- Photo Identification
- Referral (if required)
- List of all medication that you are currently taking
- Any required co-payment
- Medical and surgical history information
- X-rays or MRI’s that you may have
- Completed forms (if they have been downloaded)
Please note: all patients under the age of 18 must be accompanied by a parent or guardian
